What do I send to format my thesis?

The thesis in Word, the formatting guide approved by your university or college, and any supervisor notes.

Does formatting include automatic indexes?

Yes: the table of contents and the lists of tables and figures are built automatically from headings, so they update with any later edit.

Is the thesis language or content edited?

No. Formatting does not touch the content; proofreading is a separate service that can be ordered alongside.

How long does it take?

It depends on the thesis size and the university guide; the time is set after reviewing the file.
